The subway system is the safest location in the Lost cities - use it. The real problem is A. Getting the pickaxe and B, gathering enough bone piles for the 6 skulls. The bone piles are in houses and so you risk aggroing mobs, from traveling between houses to actually mining the skulls. You need resistance 3 buff before ever considering heading there minimum. Resistance 3 buff is obtained in 2 ways: either through a wine build. Or. using 2x resistance rings (crafted with heart of diamond) and using shield of honor. Then you need advance protection on your gear, bonus points if you also have full +2 armor rolls on your baubles.
